Information/Policy
Definitions:
Checkout information - a listing or report
of items and/or title information or materials checked out to any
individual's library card.
Policy:
Any member library user of Lakeland's automated circulation
system may have authorization to access their own checkout
information upon approval of the member library's Director.
The Cooperative has established the following guideline for
providing checkout information to patrons:
- The patron should complete and sign a Request for Checkout
Information form.
- The patron shall be provided the checkout information in a
timely manner, no later than the close of the next business day
after the Request for Checkout Information was submitted.
- Patrons may only receive checkout information for their own
library card. The library card must be presented and identity
confirmed at the time the information is provided.
